CCPD and HDGP Evaluation

Timeframe: 2015 – 2018

Purpose: Assess the impact of 17 different strategies on improving health behaviors and reducing risk behaviors that contribute to cancer, cardiovascular and chronic pulmonary disease.  The Cancer, Cardiovascular and Pulmonary Disease (CCPD) Grant Program and the Health Disparities Grant Program (HDGP) funds over 60 community agencies across the state of Colorado to implement evidence-based strategies ranging from healthy eating and active living policies, clinical systems improvement and patient navigators and community health workers. The purpose of the evaluation is to document and assess changes in healthy behaviors (e.g., physical activity, healthy eating, HPV vaccination, self-measured blood pressure) and health outcomes (e.g., diabetes, cardiovascular disease, hypertension) over a three-year period.
